Cream

Composition:
Each gram of cream contains 10mg of Terbinafine hydrochloride.
Properties:
Terbinafine is an allylamine, which has a broad spectrum of anti-fungal activity. Terbinafine interferes specifically with fungal sterol biosynthesis at an early step. This leads to a deficiency in ergosterol and to an intracellular accumulation squalene resulting in fungal cell death. Terbinafine acts by inhibition of squalene epoxidase in fungal cell membrane. The enzyme squalene epoxidase is not linked to the cytochrome P450 system. The drug has rapid onset of action and can be effective with short treatment duration.
At low concentration terbinafine is fungicidal against dermatophytes, moulds and certain dimorphic fungi. The activity against yeast is fungicidal or fungistatic, depending on the species.
Pharmcokinetics:
After topical application, less than 5% of the dose is absorbed
Indication:
Terbin® cream is indicated for the treatment of:
Fungal infection of skin, hair,nails caused by dermatophytes such as Trichophyton (e.g. T rubrum, T. mentagrophytes, T. verrucosum, and T. violaceum)
Microsporum canis and Epidermophyton floccosum.
Ringworms (tinea corporis, tinea cruris, tinea pedis and tinea capitis)
Yeast infection of the skin caused by genus candida (e.g. Candida albicans)
Pityriasis(tinea) versicolor due to Pityrosporum orbiculare (also known as Malassezia furfur)
Dosage and Administration:
The duration of treatment varies according to the indication and the severity of the infection.
Cream
Terbin®cream can be applied once or twice daily. Cleanse and dry the affected areas thoroughly before applying Terbin® cream. Apply cream to the affected skin and surrounding area in thin layer and rub gently. In the case of intertriginous infections (submammary, interdigital, intergluteal, inguinal) the application may be covered with gauze, especially at night.
Likely duration of treatment:
Tinea pedis: 1 week
Tinea corporis: 1 wee
Tinea cruris: 1 week
Cutaneous candidiasis: 1 week
Pityriasis versicolor: 2 weeks
Relief of clinical symptoms usually occurs within a few days. Irregular use or premature discontinuation of treatment carries the risk of recurrence. If there are no signs of improvement after two weeks the diagnosis should be verified.
Use of Terbin® in elderly
There is no evidence to suggest that elderly patients require different dosages or experience side effects different from those in younger patients.
Contraindications:
Hypersensitivity to terbinafine or any of the excipients contained in the cream.
Warning and Precautions:
Terbin® cream is for external use only. Ophthalmic and intravaginal use should be avoided. When irritation or sensitivity develops with the use of Terbin cream, treatment should be discontinued
Interactions:
No drug interaction is known with Terbin® cream
Side Effects:
Skin reactions: When applied locally, redness, itching or stinging occasionally occur at the site of application
Overdosage:
No case of overdosage with Terbin®cream has been reported. Acute overdosage with topical application is unlikely due to the limited absorption and would not expected to be life threatening situation.
